WAIVER FORM: If you have not been tattooed by Tanya before, or you have significant health changes to update, you must fill out a waiver form. This must be completed before your appointment.

PARENTAL CONSENT FORM: If you are 16, or 17 years old, you will need a parent to fill out the parental consent form. This must be completed before your appointment.

For groups of 3+ email kriyabeautyinfo@gmail.com with the following details for each tattoo/person: written description, placement if known, approximate size, pictures of designs, budgets, names of all friends getting tattooed. OR each fill out a tattoo request form and put in the note who you're coming with.
Each person must also fill out a waiver (and parental consent form if applicable) if they have not been tattooed by Tanya before.
